A case of Stevens-Johnson syndrome that developed from administration of antibiotics in a patient with cholestatic hepatitis

Abstract

|
Stevens-Johnson syndrome (SJS) is a rare and often lethal cutaneous drug hypersensitivity reaction, and the sufferers develop severe erythematous and vesicular or bullous mucocutaneous lesions in the skin and mucosa. In the previous case reports and studies, more than 100 drugs have been implicated as the causes of SJS, although a number of different causes exist. There have been case reports of SJS with concomitant cholestatic hepatitis, and SLS was elicited by drug (s). There has also been a case of SJS incited by drugs in a patient with underlying liver disease in the Korea. But there haven¡¯t been any case reports of SJS induced by drug, including antibiotics, in a patient with drug-induced cholestatic hepatitis. A 53-years old woman had been taking neurologic medications for an old cerebellar infarction for about a year. In the mean time, cholestatic hepatitis and leukocytopenia developed after taking the newly prescribed neurologic medications. Subsequent medications, including antibiotics, were then administered due to the development of symptoms related to the drug-induced cholestatic hepatitis. After administration of antibiotics, severe erythematous and bullous lesions emerged from her skin and mucosa, and thereafter she was diagnosed with SJS via the skin biopsy and she was treated for a long period of time. So, we report here a rare case of SJS elicited by antibiotics in a patient with drug-induced cholestatic hepatitis.(Korean J Med 71:S1157-S1164, 2006)
